Tax file number declaration (for employee's)
This form helps your payer work out how much tax to take out of payments made to you. It replaces the Employment declaration. This updated version incorporates the changes effective from 1 July 2000 in which the Commissioner extended the Tax File Number quotation exemption previously available to under-16s to all individuals under 18 earning less than the Tax Free Threshold.
The entity making the payment is your 'payer'. You are the 'payee'. You should fill in a Tax file number declaration every time you start a relationship with a payer, such as starting a job or converting superannuation entitlement into a pension. Your payer must notify the tax Office within 14 days of the start of the relationship.

Complete this form if your group certificate has been lost or destroyed, or has not been received from your employer.

Eligible termination payments
An eligible termination payment (ETP) is a lump sum superannuation benefit or similar payment. Most lump sums from a superannuation fund or approved deposit funds are ETPs. A lump sum payment from an employer to employee "in consequence of termination of employment" (other than unused annual leave, unused long service leave, tax free redundancy and early retirement payments, or salary or wages) will usually be an ETP.
